Navbar in Distributed Map is missing

In the distributed map there is no navbar.

This was fixed during the GWT updates in OpenNMS 15. Marking as fixed.

I changed the priority due to the fact that it is not a 14.0 issue anymore.

This has been this way since it was created.

I've spent most of today trying to figure out how to include the navbar in this but the technique we use in the other vaadin/gwt apps won't work here because it's a pure GWT app and we'd have to completely change the way the app gets initialized to make it work. I'd love to rework this to be sane, but changing it for 14 is going to be way too much work.
