Allow users to graph the traffic for a specific host over time.

The list of known hosts should be made available as both a template query and a dropdown in the query editor.

This issue is to track just the frontend work (helm/opennmsjs). The backend work is covered in https://issues.opennms.org/browse/HZN-1552
