Define a JMX data collection package for monitoring Cassandra 2.1.x

Description

Provide definitions for collecting performance and usage related statistics from a Cassandra cluster running 2.1.x.

The data collection package should be enabled on nodes with the category 'cassandra21x'.

We collect the graphs but the representation is not good enough to work with. Most of the graphs don't have the right unit and the naming for the graph reports are just the normal MBeans. The configuration has to be refactored regarding to the description of the task.
