This change removes all the HTML and PDF files for each adoc file. The output is now generated with for each documentation module in one index.html and one index.pdf.

The asciidoctor-maven module generates for each adoc file a HTML and PDF file. The main file for each documentation module is the index.adoc file. It contains the full content of all included files and is generated as HTML and PDF file.
