Resolved by uninstalling via yum, installing the RPM, then doing a yarn build.
Thanks to Brynjar and Jesse for troubleshooting via chat.opennms.org

Manual install via RPM put everything but the dist folder back. Doing a `yarn build` brought back the necessary files. Restarted Grafana and the app was registered.
Due to the app being removed, the data source definition had to be reentered.

After attempting a reinstall, yum failed to install. I tried installing from the RPM, but it does not populate the /var/lib/grafana/plugins/opennms-helm/dist/ folder, so it does not register with grafana properly.
I previously had release opennms-helm-1.0.0-0.20170919.64.noarch installed from the bleeding repository. I upgraded to opennms-helm-1.0.0-1.noarch in the stable repository. After the update, I no longer have OpenNMS data sources available in my Grafana server.
